Authorization for Use of Pet Photos on Social Media and Website

By signing below, I grant Stonebridge Veterinary Wellness permission to use photos or videos of my pet, taken during visits to the hospital, on the hospital’s social media platforms (such as Facebook, Instagram, etc.) and/or website for marketing, educational, and promotional purposes. I understand that these photos or videos may be shared publicly and may be used to highlight the care and services provided by Stonebridge Veterinary Wellness. I understand that only my pet's name (and not my personal information) may be used in connection with the photos or videos, unless otherwise specifically requested and consented to by me. I also acknowledge that these photos will remain the property of Stonebridge Veterinary Wellness, and I waive any right to compensation or approval of the use of the images.

I understand that I may revoke this authorization at any time by providing written notice to [Veterinary Hospital Name], but such revocation will not affect the use of images previously posted.
