This form authorizes the release of medical records as requested by the pet owner. By signing, the owner grants permission for the transfer of their pet’s medical history to a specified party, such as another veterinary clinic, specialist, or insurance provider.
This form provides consent for the use of pet photos on social media and other promotional materials. By signing, the pet owner agrees to allow the Stonebridge Veterinary Wellness to share images of their pet for marketing, educational, or community engagement purposes.
We have several payment options available to you, but please note that we are unable to offer payment plans.
Full payment is due at the time of service. In the case of a major medical procedure, we may require a deposit.
We do not accept Checks.
When unexpected illness strikes a pet, unexpected expense strikes as well. Care Credit is a line of credit for medical expenses for humans and pets. It just takes 5 minutes to complete an application. You can either apply at our hospital or apply online yourself.
