1. Acceptance of Terms
By accessing our services at Stonebridge Veterinary Wellness (“the Practice”), you agree to be bound by these Terms and Conditions, all applicable laws and regulations, and agree that you are responsible for compliance with any applicable local laws. If you do not agree with any of these terms, you are prohibited from using or accessing our services.
2. Description of Services
Stonebridge Veterinary Wellness provides veterinary care services including but not limited to wellness examinations, vaccinations, diagnostics, treatments, surgeries, and preventative care for companion animals. Our commitment to state-of-the-art premium pet care, five-star client care, and client education drives all our services.
3. Appointment Policy
3.1 Scheduling
Appointments can be scheduled by phone, through our website, or in person. We strive to accommodate your schedule but recommend booking well in advance for routine care.
3.2 Cancellations
We require 24 hours’ notice for appointment cancellations. Failure to provide adequate notice may result in a cancellation fee equal to 50% of the scheduled service.
3.3 Late Arrivals
If you arrive more than 15 minutes late for your appointment, we may need to reschedule or you may experience an extended wait time.
4. Payment Terms
4.1 Payment Due
Payment is due at the time services are rendered. We accept cash, credit cards, debit cards, and approved payment plans through third-party financing.
4.2 Cost Estimates
While we provide estimates for procedures, final costs may vary based on findings during examination or treatment. We will make reasonable efforts to notify you of significant changes to estimated costs.
4.3 Deposits
Certain procedures may require deposits before scheduling.
4.4 Unpaid Balances
Accounts with unpaid balances may be subject to collections proceedings and additional fees.
5. Medical Emergencies
During our operating hours, we will make every effort to accommodate medical emergencies. After hours, we recommend contacting our designated emergency referral partners, information for which will be provided on our answering service.
6. Medical Records and Privacy
6.1 Ownership of Records
All medical records, including laboratory results, radiographs, and other diagnostic reports remain the property of Stonebridge Veterinary Wellness.
6.2 Release of Records
Upon written request, copies of your pet’s medical records can be released to you or transferred to another veterinary practice.
6.3 Privacy Policy
We are committed to protecting your privacy. Information collected during the provision of veterinary services will be used solely for the purpose of providing care to your pet and will not be shared with third parties except as required by law or with your explicit consent.
7. Client Responsibilities
7.1 Accurate Information
Clients must provide accurate and complete information regarding their pet’s health history, current medications, and any observed symptoms.
7.2 Pet Control
Pets must be properly controlled at all times while on the premises. Dogs should be on leashes and cats or small animals should be in appropriate carriers.
7.3 Compliance with Treatment
Following our treatment recommendations and medication instructions is essential for your pet’s well-being. Non-compliance may impact treatment outcomes.
8. Liability Limitation
8.1 Standard of Care
While we strive to provide the highest quality care in line with our core values of ethics and state-of-the-art premium pet care, we cannot guarantee specific outcomes for treatments or procedures.
8.2 Limitation of Liability
Our liability is limited to the cost of services provided. We are not liable for any indirect, consequential, or special damages arising from our services.
9. Termination of Care
We reserve the right to terminate the veterinarian-client-patient relationship due to non-compliance with treatment recommendations, abusive behavior toward staff, or non-payment for services.
10. Dispute Resolution
Any dispute arising from our services shall first be addressed through direct communication. If resolution cannot be reached, disputes shall be resolved through mediation or arbitration in Placer County, California.
11. Changes to Terms and Conditions
We reserve the right to modify these Terms and Conditions at any time. Changes will be effective immediately upon posting to our website or premises. Continued use of our services constitutes acceptance of the modified terms.
12. Governing Law
These Terms and Conditions shall be governed by and construed in accordance with the laws of the State of California, without regard to its conflict of law provisions.
